What is Transitioning at Magic Life?
Transitions occur throughout life and are faced by all young people as they progress, from childhood through puberty and adolescence to adulthood; from immaturity to maturity and from dependence to independence.
In addition, some young people experience extra transitions because of other life events, for example, bereavement, separation of parents, and being placed in care.
Transitioning in a supported living environment.
For a young person with a learning disability entering adulthood, it is essential that a person-centred designed programme of support for facilitating a smooth transition is planned from the onset and from early as possible.
At Magic Life our assessment process is quite unique in that we carry out several assessments in different settings, we carry out your first assessment at the current place you are living, we would then carry out another assessment within the community and a final one in your proposed accommodation, if further assessments are needed we put this in place. If agreed that you will be moving into one of Magic Lifes properties a transition plan is put in place which is done over a four-week period and would usually be along the lines of the following:
